FOUR STEPS TO IMMUNIZATION COMPLIANCE

Hi, I’m Indya and I’m here to show you how to make sure you’re immunization compliant in time for registration. It’s an easy four-step process.

Step One: Make sure you have a signed and verified immunization record. 
If you don’t, start by logging into OPUS with your Emory Net ID username and password at O-P-U-S dot Emory dot E-D-U. Click on the tasks tile to see your personal “To-Dos List.” 

Look for your immunization related to-dos, follow the links to the Student Health Patient Portal. Sign in with your Emory Net ID and password again. Click on the immunization form link listed on the Emory Patient Portal home page. Download and print your school-specific immunization form. Have your doctor complete and sign it. Keep the signed form handy! You’ll need it later for step three.

Step Two: Complete the tuberculosis questionnaire.
Go back to the Emory Patient Portal home page. Click on the tuberculosis questionnaire link and fill it out.

Step Three: Enter your immunization dates online.
Return to the Emory Patient Portal home page. Click on the Immunizations Form link. Scroll down and enter all your immunization dates. Hit “Submit.” You’re almost done! 

Step Four: Upload your signed and verified immunization records.
Go to the bottom of the immunization form where you hit submit to upload a healthcare-provider signed and verified copy of your immunization record.

Cuatro pasos para cumplir con las inmunizaciones

Hola, me llamo India y estoy aquí para mostrarte como cumplir con las inmunizaciones a tiempo para tu registro de clases. Es un proceso fácil con cuatro pasos.

Primer paso: Asegúrate de tener un registro de inmunización verificado y firmado.
Si no lo tienes, puedes empezar por ingresar a OPUS con tu usuario de Emory NETID y contraseña. E n opus.emory.edu dale click a la ficha de tarea para ver tu lista personal de tareas. Busca las tareas de inmunización. Sigue los enlaces al portal de pacientes de Student Health. Ingresa con tu usuario de Emory NETID y contraseña de nuevo.
Dale click al enlace del formato de inmunización en la página de inicio del portal de Pacientes de Emory. Descarga e imprime el formato de inmunización específico de tu facultad. Preséntale el formato a tu doctor para que lo complete y lo firme. Mantén una copia del formato contigo – la vas a necesitar después para completar el tercer paso.

Segundo paso: Completa el cuestionario de tuberculosis
Regresa a la página de inicio en el portal de pacientes de Emory. Dale click al link del cuestionario de tuberculosis y complétalo.

Tercer paso: Ingresa tus fechas de inmunización en línea
Regresa a la página de inicio en el portal de pacientes de Emory. Dale click al link del formato de inmunización. Navega hasta abajo e ingresa todas las fechas de inmunización. Presiona el botón para enviar. Bueno, ya casi terminas!

Cuarto Paso: Sube tus registros de inmunización firmados y verificados
Ve a la parte de abajo del formato de inmunización en el que presionaste el botón de enviar para subir la copia de tus registros de inmunización firmada y verificada por un proveedor de servicios médicos.

Una vez Emory Student Health haya verificado tus registros, OPUS removerá las tareas de inmunización de tu lista de tareas y estarás en cumplimiento con las inmunizaciones. La verificación puede tomar hasta cinco días hábiles así que planea como corresponde.

여러분 학과에서 요구한 면역 서식을 다운로드 받아 프린트해주세요. 여러분의 의사가 작성하고 사인하셔야 합니다. 인증받은 서식을 잘 보관해두세요! 3단계를 위해서 필요하거든요.

2단계: 결핵 질문지를 작성해주세요.
Emory Patient Portal 홈페이지를 다시 가주세요. 결핵 질문지 링크를 클릭하셔서 작성해주세요.

3단계: 예방접종 한 날짜를 입력해주세요.
Emory Patient Portal 홈페이지에 다시 가주세요. 면역 서식 링크를 클릭해주세요. 모든 예방접종 날짜를 기재해주세요. Submit 버튼을 눌러주세요. 자 그럼, 거의 다 하셨습니다!

4단계: 서명되고 인증받은 면역 기록을 업로드해주세요. 
의료진이 서명하고 인증한 여러분의 면역 기록을 업로드하기위해 면역 서식 제일 하단부로 가주세요.

일단 Emory Student Health가 여러분이 기록을 확인하면, OPUS는 면역 임무를 여러분의 to-do 리스트에서 삭제할 것이고, 여러분은 면역 규정을 준수하시게 됩니다. 확인절차는 평일 기준 5일까지 걸릴 수 있으니 거기에 따른 시간상 계획을 해주세요.

4部上传疫苗接种纪录 

大家好，我是Indya。我来为大家讲解如何一步一步填写和上传疫苗纪录。分4部：

其中关于疫苗的任务里面有连接可以进入到Student Health Patient Portal。登录，在主页左侧选择Form， 选择其中Immunizations Form里面有学校需要的疫苗。如果没有疫苗本的话可以下载需要的文件让医生填写。再上传。

第二部：上网填写肺结核问卷（tuberculosis questionnaire）
上Emory Student Health Patient Portal，登录，在主页点击Tuberculosis Questionnaire并填写。

第三部：网上输入疫苗接种日期 
上Emory Student Patient Portal，登录，在主页左侧选择Form。填写。再选择其中Immunizations Form然后填写日期。

最后一步。

第四部：上传所有疫苗接种纪录
在第三部的Immunizations Form里，上传所有需要的疫苗接种纪录。
