How do I request a copy of my medical record?

To submit the request electronically, you must be a registered student with a current OPUS account. Click on any "Your Patient Portal" icon on this web site, log-in, and select "messages" from the left menu bar.  Next, select: "Request a Release of your Personal Healthcare Records to another healthcare provider or an individual, including yourself." Fill in the requested information, and submit your request.

If you have already graduated or left school: You can submit your request by email, mail or fax. Click here and print off the "Medical Records Release Form." Complete the form, then either mail, fax, or email/attach your request:

  • To submit this request electronically, send attach a completed PDF version of the release form to  Please note that we cannot release medical information electronically.

Who is authorized to sign for the release of information to obtain a copy of the medical record?

The following individuals may sign the authorization for release of medical information:

  • The student/patient (but not the student's spouse)
  • Parent (only if the student/patient is under the age of 18)
  • An individual with authorized Power of Attorney if the student/patient is not able to sign (documentation must be provided)

Please note: a spouse or domestic partner many not request medical records without the student/patient's consent.

How much does it cost to obtain a copy of my medical record?

There is no charge for releasing medical records directly to enrolled students. Former students will pay a fee for medical record copies ($27) or immunization records only ($10).

When will I receive a copy of my medical record?

The Medical Records Department receives a large number of requests and many of the records requested are very complicated. Your request will be processed within 5 business days. If you need your records more urgently, please call the Medical Records Department at 404-727-7304.
