The Emory University Student Health Insurance Plan (EUSHIP)

Emory University requires that all degree-seeking and all international students either have health insurance or purchase the University-sponsored plan. In order to meet this requirement, you must either choose to enroll in the Emory University Student Health Insurance Plan (EUSHIP) or successfully complete the waiver process on-line. If you choose to enroll in the EUSHIP, you will be charged via Student Financial Services. 1095-B Forms

You will receive your 2018 1095-B tax form from Aetna Student Health for coverage through Jan 1, 2018 – Jul/Aug 14, 2018 under the Aetna fully insured plan. If you have not received your 1095 from Aetna, contact Aetna Student Health at 1-855-531-6837 or access your Aetna Navigator at www.aetna.com, for an electronic copy,

Emory University will provide you with a 1095 tax form for coverage through Aug 15, 2018 – Dec 31, 2018 under the Emory self-insured plan. If you have not received your 1095 form from Emory, contact Core Management Resource Group at 478.741.3521 ex:150 or email them at:  chandler.wagoner@corehealthbenefits.com
